• The Great Lakes Nutrition referral process is simple: below you will find our commercial insurance referral form and our Medicare insurance referral form.

• The referral form can be faxed to 833-222-8309.  All lab work and current diagnoses are helpful in securing maximum insurance coverage. 

Great Lakes Nutrition accepts Aetna, Anthem, BCBS, Cigna, Highmark BCBS, Medical Mutual Supermed PPO, Medicare, Meritain, self pay and UHC.

NOTE: We do not take any Medicaid or Tricare products at this time.
